All students borrowing through the Federal Perkins Loan program at The College of St. Scholastica, must complete step #1. Only students packaged with the Federal Perkins Loan are eligible to apply for the loan.

Step #1: Print and complete the Perkins Confirmation Form.

• This form should be returned to the College of St. Scholastica, Financial Aid Service Center-Tower 1118, 1200 Kenwood Avenue, Duluth, MN 55811 or by fax (218) 733-2229.

If you are a FIRST time borrower of the Federal Perkins Loan at The College of St. Scholastica, you must ALSO complete steps #2 &3.

Step #2: Complete Loan Entrance Counseling.

Step #3: Complete the Master Promissory Note.
