Health Science Majors
Those students who are accepted into certain health science majors: Nursing, Occupational Therapy, Physical Therapy, Health Information Management, or Exercise Physiology will be required to provide additional immunization information. Please indicate the month, day, and year of administration.
- Td or Tdap: Given within the last 10 years. The new Tdap vaccine, released 6/05 for adult use, is recommended for anyone who's last Tetanus booster was at least 2 years ago. Some health care facilities (St. Mary's Hospital) require Tdap for students doing clinical work in their facilities. Required for nursing.
- MMR: Two dates of administration after 12 months of age.
- Hepatitis B: The dates of 3 immunizations.
In addition to these dates each program has other specific requirements.
Nursing: Medical exam form completed and turned in to health service at time of application to the program, including the month and year of either chicken pox disease or dates of Varicella immunization(s). Annual mantoux testing sophomore, junior and senior years, provided at no charge by Health Service. Annual Health Update form.
PT/OT: Annual Health screening (provided at no additional charge in health service). The first year of the program laboratory evidence of immunity to rubella and varicella illness is required. If done at the time of your health screen in Student Health Service, these lab tests will be billed to your health insurance company. The necessary form is available in the program office and in Student Health Service. Annual mantoux testing annually, provided at no charge by Health Service.
