Last name first, then first and middle names. Avoid the use of initials. Follow it with a comma and dir. - Hitchcock, Alfred, dir.


Title of the movie
Underlined followed by a period - Suspicion.


Names of major performers in movie
List the names of a couple of major actors/actresses in the movie preceded by Perf. and followed by a period - Perf. Cary Grant and Joan Fontaine.


Date of the original release of the movie
Give the year of when the movie was originally released followed by a period - 1941.


Medium in which you watched the movie
This would the format you used to watch the movie such as DVD, Videocassette, Laser Disc, etc. followed by a period - DVD.


Name of the distributor
Give the name of the company that released the movie such as Paramount Pictures, Lucas Films, Walt Disney, etc. followed by a comma - Turner,

Date of version you watched
Give the year of when the version you watched was released followed by a period - 1995.
