Required Documentation

The documentation of a student's disability must be precise, related to the requested accommodations, and be provided by a qualified/licensed professional.

Psychological disabilities, learning disabilities, and AD/HD must be include a DSM-IV diagnosis and results of the tests administered. A physical disability diagnosis must include the instruments used to determine the degree of limitations.

For documentation to be considered current, the qualified professional's statement must be:

  • Less than 1 year old for a psychological disability
  • Less than 3 years old for all other disabilities

***Individualized Education Plans (IEP's) and 504 Plans are not sufficient forms of documentation on the postsecondary education level***
