Required Documentation The documentation of a student's disability must be precise, related to the requested accommodations, and be provided by a qualified/licensed professional. Psychological disabilities, learning disabilities, and AD/HD must be include a DSM-IV diagnosis and results of the tests administered. A physical disability diagnosis must include the instruments used to determine the degree of limitations. For documentation to be considered current, the qualified professional's statement must be: - Less than 1 year old for a psychological disability
- Less than 3 years old for all other disabilities
