MyHealth

MyHealth is Student Health Service's portal for our electronic health records system. Via MyHealth, students can submit their immunization and health history, send messages to health care providers, request prescription refills, complete release of information requests, view and print immunization records and much more. MyHealth is accessible through Cor, on the Support Services page, or directly at https://myhealth.css.edu.

Required Form Completion

Vaccination History Form:

Per Minnesota State Law (M.S. 135A.14) students (excluding Online and Extended Studies) must provide their vaccination history to The College of St. Scholastica. Students must complete the Vaccination History Form via MyHealth inorder to provide this information. Student Health Service no longer accepts immunization records on paper.

Exemptions:

 For students who would like to claim Age Exemption (for those born before January 1957) or who would like to claim Minnesota High School Graduate Exemption (for those who graduated from a Minnesota high school after January 1997), please log into your MyHealth account, select the 'Messages' tab on the left side of the page, click the 'New Message' button, select the appropriate exemption, completely fill out the form and then hit the submit button.

Students who would like to claim a Medical Exemption should print, complete, and return the Medical Exemption Form.

Students who Conscientiously Object to vaccinations should print, complete, and return the Conscientious Exemption Form.

 

Notice of Prvacy Practices and Agreement to Medical Treatment:

Prior to being seen in the Student Health Service office, students must acknowledge both the Notice of Privacy Practices and Agreement to Medical Treatment forms. 

Health History Form:

Prior to health screens (for Exercise Physiology, Health Information and Informatics Management, Occupational Therapy, and Physical Therapy academic programs) and physicals (Athletic, Well Women, etc), students must complete the Health History Form.
