Dear College of St. Scholastica Student and Parents:

The College of St. Scholastica is vitally interested in the health and welfare of our students. As a result, we believe that we have found the best possible student health insurance plan available at a rate that is affordable to all students.

Again this year we are working with USI Affinity Collegiate Insurance Resources, formally Specialty Risk Group, a leading broker in Student Accident and Sickness plans. Together we have enhanced the plan by expanding the benefits to better meet the needs of our students. The plan is underwritten by Combined Insurance Company of America in association with Administrative Concepts, Inc. The cost of the insurance is $491.00 per term (term dates are listed in the brochure) and will provide coverage for a full twelve months from August 2, 2009 to August 2, 2010.

Voluntary athletic coverage is available if an athlete is enrolled in the traditional student health insurance policy.  Premium per sport for basketball, football, ice hockey, skiing, soccer and softball is $141 per athlete per sport.  Premium per sport for all other sports and club sports is $71 per athlete per sport.  This is annual coverage and is not pro-rated.

If you have adequate coverage through another insurer, please complete the online waiver by July 15th to ensure an accurate Fall Schedule/Bill. Log on to your Banner Web account using the link on the right column. Click on Personal Information, Authorization/Waiver Form and complete the Waive 2009-10 Student Health Insurance.

September 25th is the final deadline to waive insurance if you do not wish to be enrolled in the plan.

Family coverage is available to students enrolled in the insurance plan. You can inquire about family coverage at this website http://www.collegiateinsuranceresources.com/sip/index_copy.asp  Students taking less than twelve credits may enroll in the health insurance at this website as well.

If you do not have access to the Internet from home or a public library, you can waive the insurance coverage by using a computer on campus prior to the September 25th deadline.

We strongly encourage you to read the online brochure at http://www.collegiateinsuranceresources.com/sip/plan_details.asp. Please note that many home based HMO plans do not provide adequate coverage outside of your home area. Therefore, if you consider waiving this plan, we recommend that you review your current health insurance policy to ensure that it adequately covers you while pursuing your academic interests at The College of St. Scholastica.
