• Working Portfolio Evidence Form
  Form is used for submitting an artifact for standard/ disposition/program outcome.
• INTASC Standards/ EDU Dispositions Alignment
  This form identifies the alignment of the Education Department's Dispositions for Educators with the 10 INTASC standards, to assist students and teachers see the correlation between and among them.
• INTASC Standards with Indicators
  Listing of INTASC Standards ofEffective Practice with indicators.
• Field Experience Student Placement Survey
  This survey is completed by students early in EDU 2505 and EDU 3505 and is designed to give students input into their field experience placement. It is also used by placement coordinators to help assure that placements cover the full range of the K-8 license.
• Student Background Summary
  This survey is used as a guide for students as they document background information that will be shared with cooperating teachers.
• Field Experience Lesson Planning Guidelines & Form
  IN PDF FORMAT Provides students with a suggested format as they develop lessons to be taught in the various field experiences.

• Lesson Observation Report
  This lesson evaluation is completed by the cooperating teacher or college supervisor.
• Lesson Observation Rubric
  Use as a guide when filling out Lesson Observation Report while observing the teaching of preservice teachers.
• Field Experience Time Log
  Students complete the log as a verification of time spent working in the classroom. It must be signed by the cooperating teacher and submitted to the course instructor at the end of the semester.
• EDU 1505 Introduction to Teaching Time Log
  Students use this form to document their school activities and keep track of time spent in the classroom.

• EDU 2505 Field Experience: Communications/ Classroom Management: Final Performance Checklist
  This is an evaluation checklist of MNTASC licensure standards to be attained during this course. It is completed by the cooperating teacher and forwarded directly to the college. This form is completed by the cooperating teacher. It is used as an evaluation of the student's progress in this field experience.

  This form is used to provide performance feedback to CSS supervisors and to field experience students. It is completed by the cooperating teacher.
• EDU 2505, 2805, 3505, Content Areas: Final Evaluation
  This form is completed by the cooperating teacher. It is used as an evaluation of the student's progress in the field experiences listed above.
• EDU 3505 Field Experience: Content Areas/Classroom Management: Final Performance Checklist
  This is an evaluation checklist of MNTASC licensure standards to be attained during this course. It is completed by the cooperating teacher and forwarded directly to the college. This lesson evaluation is completed by the cooperating teacher or college supervisor.
• High School Daily Lesson Evaluation
  This form is completed by the cooperating teacher. It is used as an evaluation of the student's progress in this field experience.
• Student Teaching Mid-Term Evaluation
  This form is used to provide performance feedback to CSS supervisors and to student teachers. It is completed by the cooperating teacher.
• Student Teacher Final Evaluation
  This evaluation is be completed and signed by the cooperating teacher during the last week of student teaching. The cooperating teacher writes a letter of recommendation for the student teacher and submits it with the evaluation.
