• EDU 4500 Elementary Student Teaching Standards Assessment
  This form is used during elementary
  student teaching. It assesses the
  Standards of Effective Practice,
  which are embedded in the course.
• EDU 4700 Secondary Student Teaching Standards Assessment
  This form is used during secondary
  student teaching. It assesses the
  Standards of Effective Practice,
  which are embedded in the course.
• Professional Behaviors Form
  This is an evaluation of a students Professional Behaviors. It is to be
  completed by the cooperating teacher
  and forwarded directly to the college.
• Context for Learning Form
  Used as required in your field
  experiences. Students will include information on demographics,
  community context, physical
  environment, instruction and
  learning environment.
