Lesson Plan Design Framework

The field experience student will use this framework to design lessons for all formal observations by the cooperating teacher and college supervisor.

The field experience student will use this form to design lessons for all formal observations by the cooperating teacher and college supervisor. A copy of the lesson plan should be provided for the cooperating teacher and college supervisor prior to the formal observation.

Lesson Observation Form

The observation form is to completed for each formal observation completed by the cooperating teacher and college supervisor. Copies of the completed observation form should be given to the student teacher and the college supervisor.

Field Experience Time Log

This form is used for field experience students to record their hours in the classroom and activities that they were involved with during these field experience hours. This form must be signed by the cooperating teacher after each visit.
